St. Joseph Hospital West’s Senior Services offers assistance to seniors and long term care residents of St. Charles, Lincoln, and Warren counties.

We provide information and referral resources for patients, caregivers and their adult children about aging issues through services at St. Joseph Hospital West.

Coordination of Care for Patients in
Long-Term Care Facilities

The SSM St. Joseph Senior Services Outreach Nurse:

  • acts as a liaison between our hospitals and the area long-term care facilities.
  • facilitates the relationship between the hospital and long-term care facilities.
  • will coordinate with the Senior Services staff, acute care nursing staff, case managers and social workers.
  • follows up with the long-term care facility’s staff to insure a smooth transition from our hospital.
  • is an important link in maintaining a continuum of care in the best interest of the patient.
Patient Ambassadors:
  • a team of volunteer “ambassadors” who visit older acute care patients.
  • visit patients on the day of admission to assist in their transition, answer questions and help solve problems.
  • act as a channel of communication for the patient, their family members and the various hospital staffs.
Lifeline - Personal Response and Support Services:
  • Lifeline, a personal response and support service, gives older adults the confidence and freedom to live on their own, in their own homes.
  • features a lightweight, waterproof help button and a small in-home unit called a communicator.
  • In the case of emergency, this system puts the older adult in touch with a caring Lifeline Monitor who will obtain the help needed immediately.
