Referral Process

• A signed doctor's order facilitates insurance reimbursement. The order should include diagnosis code, medications, pertinent lab values and specific instructions for blood glucose monitoring.

Insurance Coverage

• Coverage varies. Patients are encouraged to check with their insurance provider for benefit information.
• Diabetes Self-Management Training (DSMT) and Medical Nutrition Therapy (MNT) are Medicare benefits.
